Text Messaging and Electronic Communications Terms

Effective Date: [EFFECTIVE DATE]

These terms describe how [PRACTICE LEGAL NAME / DBA] may communicate by text message, telephone, email, patient portal, or other electronic means. Specific consents presented during registration or within the patient portal may provide additional terms.

Types of Communications

  • Care and operational communications: appointment reminders, scheduling, registration, care coordination, laboratory or prescription follow-up, billing, payment, portal notifications, and other communications related to care or practice operations.
  • Patient education and informational communications: health information, service updates, office announcements, and educational materials.
  • Marketing communications: promotional messages sent only when permitted by applicable law and any required consent.

Consent and Choice

By providing a telephone number or email address, you represent that you are authorized to use it and consent to communications permitted by law and your selections. Consent to marketing texts or emails is not a condition of receiving health care. You may revoke consent using a reasonable method described below.

Text Message Terms

  • Program name: [SMS PROGRAM NAME].
  • Messages may be sent from [SMS SENDING NUMBER] or another number used by the practice or its authorized service provider.
  • Message frequency varies based on appointments, care, account activity, and preferences.
  • Message and data rates may apply.
  • Reply [STOP KEYWORD] to stop recurring texts from the applicable program. Reply [HELP KEYWORD] for help.
  • A confirmation message may be sent after an opt-out request. Processing may take a reasonable period as permitted by law.

Email Terms

Email may be used for operational, educational, or marketing communications as permitted by law. Commercial or promotional email will include a method to unsubscribe where required. An unsubscribe request may not stop nonmarketing messages necessary for care, safety, billing, legal notices, or an existing transaction.

Privacy and Security Limitations

Standard text messages and email may not be encrypted and may be seen by anyone with access to your device, account, carrier records, lock-screen notifications, or shared telephone or email address. Do not send highly sensitive information through ordinary text or email. Use the secure patient portal when instructed.

Not for Emergencies

Electronic messages are not continuously monitored and should not be used for emergencies, urgent symptoms, or time-sensitive safety concerns. Call 911 for emergencies. Call or text 988 for a mental-health or suicide crisis in the United States.

Delivery and Number Reassignment

Delivery is not guaranteed and may be delayed or blocked by carriers, devices, filtering, internet outages, or incorrect contact information. Tell the practice promptly if your telephone number or email address changes or is reassigned.
