Billing and Financial Responsibility
At Bluestem Family Medical, we believe healthcare costs should be as transparent and straightforward as possible. Our team is committed to helping patients understand their insurance benefits, financial responsibilities, and available payment options.
Patients are responsible for all applicable co-payments, deductibles, co-insurance amounts, and any services not covered by their insurance plan. As a courtesy, Bluestem Family Medical will submit claims to participating insurance carriers; however, coverage decisions and payment determinations are made solely by your insurance provider.
Payment is expected at the time services are rendered unless prior arrangements have been made. We accept major credit cards, debit cards, HSA/FSA cards, and other approved payment methods.
Certain services may not be covered by insurance and may require self-payment, including:
- Weight management services or medications not covered by insurance
- Administrative forms, letters, or paperwork requests
- Medical marijuana evaluations and certifications
- Other non-covered services as determined by your insurance carrier
For uninsured and self-pay patients, transparent pricing information is available upon request. In accordance with federal law, eligible patients may request a Good Faith Estimate outlining the expected cost of services prior to receiving care.

Out-of-Network & Self-Pay Patients
Bluestem Family Medicine welcomes both insured and self-pay patients. For individuals who are out-of-network or choosing services not covered by insurance, we offer transparent self-pay pricing and personalized treatment planning designed around your goals and needs. Many of our advanced wellness, functional medicine, performance optimization, and specialty mental health services may not be eligible for insurance reimbursement. Our team is happy to discuss expected costs, available payment options, and any recommended services prior to treatment so you can make informed decisions about your care.

Insurance & Billing Disclosures
Insurance coverage and reimbursement are determined solely by your insurance carrier and individual plan benefits. While Bluestem Family Medical submits claims as a courtesy whenever applicable, patients remain financially responsible for any co-pays, deductibles, co-insurance amounts, non-covered services, or balances not paid by insurance.
Patients may request an itemized statement or superbill for services rendered. Certain wellness, functional medicine, performance, aesthetic, or specialty services may be considered elective or non-covered by insurance plans and may require payment at the time of service.
